Diabetes Service

The THS Diabetes Service focuses on the need for better management and education of a targeted group of people with diabetes to achieve better health outcomes.

Service Objectives:

  • Improve the quality of diabetes healthcare for Pacific people with diabetes.

  • Acknowledge the cultural values and beliefs and the involvement of the family and community in providing effective diabetes services to Pacific people.

  • Ensure ongoing monitoring, evaluation and the subsequent improvement in the quality of diabetes healthcare services for eligible population.

Service Area:

  • Intensive diabetes management for newly diagnosed, patients

  • Initiation of insulin for Type 2 patients, insulin titration and change in Insulin

  • People with poor diabetic control, i.e. HbA1c>64mmol/mol

  • Nutrition advice and foot care advice (not otherwise provided in dietician and podiatry)

  • Specific clinical indicators like microalbuminuria, Blood Pressure>140/80, Total Cholesterol >9

  • Monitor and manage some complications, referring where appropriate, to specialist services

  • Undertake CVD risk assessment and management
